Instructions for Mac OS X

Right-click (Command-click) on the desktop and create a new folder

Name the folder “Patient Name” (eg Jane Doe)

Place copy of the scan in the folder created in Step 1 – either export scan in DICOM format into the folder, or copy the contents of the scan CD/DVD into the folder

Right click (Command + Click) on the folder created in Step 1

Select “Compress Patient Name” from the contextual menu

You’re done!  A Zip File called “Patient Name.zip” has been created on your desktop – please upload this file after submitting patient details via the Request Form
